Application for Employment: To download and print an official CCBDD employment application, use the link below. If you already have an application on file with CCBDD that was mailed less than 12 months ago, please do not resubmit a new application unless key information has changed. If you have a current application on file, all you need to do is send a letter of interest to the CCBDD employment manager and indicate that you already have an application on file. Note that all letters of interest and employment applications must be sent directly to the employment manager in Human Resources.

Licensed Physical Therapy Assistant Part-Time (TBD) ...download the pdf
Promote optimal level of function to individuals with DD through various appropriate therapeutic techniques within guidelines as established by Ohio law and rules of practice and professional code of ethics and philosophy of DD.
Adult Program Specialist (SWR) ...download the pdf
Supervise and train an adult or a group of adults with moderate to profound developmental disabilities in work and habilitative activities to maximize normalization. Adults may exhibit special behavioral, emotional, or daily living problems.
Community Employment Specialist (NCR) ...download the pdf
Supervise, direct and train a group of adults with developmental disabilities in community-based work settings.
